Job Description

Position Summary

Lionheart Wellness & Recovery is seeking a compassionate, detail-oriented professional to join our team in a dual role that provides both Administrative and Treatment Coordination support. This position plays a key role in ensuring that clients receive consistent, 245G-compliant, trauma-informed care while our program runs smoothly day-to-day.

This hybrid role includes:

  • 50% Administrative Assistant duties supporting admissions, DAANES, documentation, front desk operations, and program flow.
  • 50% Treatment Coordination duties supporting the clinical team under MN 245G.07 and 245G.11.

Lionheart provides all required training and supervision to help you meet competency requirements.

Administrative Assistant Responsibilities

You will support the program by:

  • Completing DAANES reporting and maintaining accuracy per DHS requirements.
  • Supporting the Admissions process: intake paperwork, insurance verification, appointment scheduling, and EHR setup.
  • Managing reception duties, client check-ins, and phone coverage.
  • Maintaining organized digital client files.
  • Providing daily operational support to clinical and leadership staff.
  • Assisting with tracking referrals, scheduling assessments, and communicating with clients and external partners.
  • Performing clerical tasks such as scanning, data entry, reminders, and supply management.

Treatment Coordinator Responsibilities

Under supervision, the Treatment Coordinator assists with tasks required to support client treatment and compliance with MN 245G, including:

  • Helping facilitate continuity of care by coordinating with:
  • significant others
  • family members
  • child welfare & social services
  • probation and legal professionals
  • medical and mental health providers
  • housing providers or sober living environments
  • Assisting with referrals, arranging appointments, gathering collateral information, and exchanges of information per consent.
  • Assisting clients in obtaining necessary resources and services such as financial assistance, housing, food, clothing, medical care, education, harm reduction services, vocational support, and recreational services that promote recovery.
  • Helping clients connect and engage with self-help support groups and expand social support networks with family, friends, and organizations
  • Supporting transitions including discharge planning, community resources, recovery support, and follow-up services.
  • Support the clinical team in gathering information across the ASAM 6 dimensions.
  • Monitor client progress toward goals and communicate updates during multidisciplinary team meetings.
  • Help ensure treatment coordination activities are documented accurately and on time.
  • Collaborate closely with counselors to ensure 245G-compliant documentation and follow-up.
  • Participate in clinical team meetings, trainings, and supervision sessions.

Lionheart will provide all required supervision, training, and competency development needed to meet 245G expectations.

Qualifications

Required:

  • High school diploma or GED
  • Strong organizational and communication skills
  • Ability to maintain confidentiality in compliance with HIPAA
  • Basic computer proficiency and willingness to learn our EHR (Ritten)

Preferred:

  • Bachelor’s degree in behavioral health, human services, psychology, social work, counseling, or related field
  • Experience in healthcare, treatment programs, social services, office administration, or customer service
  • Familiarity with Minnesota DHS, DAANES, or 245G programs (not required)
