We are seeking an exceptional Pharmacist to lead our Chronic Care Management (CCM), Transition Care Management (TCM), and Remote Patient Monitoring (RPM) services. This full clinical service program will operate through a Collaborative Practice Agreement with independent medical facilities in the Elizabeth City, NC area.
As a Clinical Pharmacist, you will establish and maintain professional relationships with all team members within the medical practice. Responsibilities may vary depending on the unique needs of each independent practice, offering you a diverse and stimulating workload. Must live in a commutable distance to the posted job location – this role is performed in provider clinics and may require regional travel to a group of clinics.
