Pay Range*: $34.79/hour - $50.42/hour

Position Summary

Under minimal supervision and in a professional and friendly manner, perform assigned job duties to the highest level of customer service and assistance to both internal and external customers of Memorial Care Medical Foundations Inpatient/Specialty Services unit.

Essential Functions and Responsibilities of the Job

1. On-site, telephonic or faxed clinical reviews (admission and concurrent) for MCMF patients in identified settings.

2. Document appropriately and per regulatory requirements within aligned CM system.

3. Provide discharge planning per assignment and in accordance with Health Plan and DOFR.

4. Support after hours duties for inpatient workflow if assigned

5. Be at work and be on time.

6. Follow company policies, procedures and directives.

7. Interact in a positive and constructive manner.

8. Prioritize and multitask.

• Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements

Experience

• Work experience in Microsoft Word, Microsoft Excel and Microsoft Outlook.
• Work experience applying evidence-based criteria (i.e.: MCG, lnterqual); Health plan medical policy / clinical coverage guidelines.
• Previous inpatient Case Management experience.
• Previous and/or ability to learn use of electronic medical record, UM/CM platforms or electronic health record.
• Knowledge of CMS (Centers for Medicare and Medicaid Services) coverage and criteria.

Education

• Associate's degree (AA) in nursing or equivalent from a two-year college or technical school.
• Current California LVN/LPN.
