Job Description

Description Summary: This position is responsible for providing care to assigned patients that meets their physical, psychological, social, cultural, spiritual, and developmental needs. Functions as a contributing member of the health care team. Responsibilities: Assists the team leader in assessing the status of patients with simple health problems on admissions and on a continuing basis reporting changes in health status to the RN and / or Physician. Contributes information to the plan of care. Implements the plan of care, under the direction of the RN and / or Physician and within the scope of practice, to meet the basic needs of assigned patients and families. Documents data for assessments, interventions and effectiveness of care provided in the patients medical record. Findings are reported to the RN and / or Physician. Participates in patient education activities as directed by the RN and / or Physician. Assists in the preparation of the patient and family for discharge under the direction of the RN and / or Physician. Administers medications to assigned patients in accordance with clinical specific standards. Responds to emergencies. Identifies safety hazards and reports them. Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the age specific and developmental needs of patients served by the department. Appropriately adapts assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, spiritual, age specific and other developmental needs of each patient served. Demonstrates adherence to the CORE values of Santa Rosa Health Care. Perform other duties as assigned. Requirements: Education High School Diploma or GED Required Graduate from an accredited school of vocational nursing Experience Experience in clinical setting preferred Licenses, Registrations, or Certifications Licensed as a LVN in Texas BLS Certification required Work Schedule:

8AM - 5PM Monday-Friday Work Type: Full Time
