We are looking for a Temporary Fulltime, Patient Care Coordinator for the Covina Home Health branch.
LVN license is required.
$30- $34 per hour

Who you are:

• Prepares nursing call schedule.
• Receive updated orders from physicians
• Takes and prepares intake referrals.
• Follows up on patient grievances.
• Participates in team and goals of the Agency.
• Participates in weekly case conferences when available.
• Requests authorization of utilization from third party payors.
• When requested, gives a report to case managers of third-party payors.
• Performs nursing care and other field staff functions as necessary.
• Other duties as assigned.
